In the event your child becomes ill or needs over-the-counter medications or prescriptions, Blair will obtain any necessary supplies or medical devices from North Warren Pharmacy. Contact North Warren Pharmacy directly at (908) 362-5156 to set up your account and provide a credit card or debit card number for this purpose, which will be stored confidentially in your child’s pharmacy record. if you prefer to share that information in person, by mail or by fax, the pharmacy’s physical address is 155 State Route 94, Blairstown, NJ 07825, and the fax number is (908) 362-5896.
Do you need to print a child's medical forms?
ONLY IF any of the following forms apply to your child, please print and bring them with you to his or her physical for your healthcare provider to complete and sign. (These forms would be necessary if, for example, your child is taking prescription medications or controlled substances or if he or she requires an action plan for asthma, allergy, diabetes or seizures.)
Can North Warren Pharmacy dispense drugs without payment?
It is very important to keep your payment information current, as the pharmacy will not be able to dispense any medications/supplies without payment. We do not want your child to have to wait for medication until someone at the pharmacy reaches you. All prescriptions for controlled substances must be blister packed; no exceptions. North Warren Pharmacy is able to provide this service.
