All forms are due to the health office by June 4, 2021. Campers will not be allowed to remain at camp if they arrive on the first day of their program without up-to-date, completed, and signed forms. All camper forms and forms from doctor's offices must be submitted in English. At this time we are unable to accept forms in any other languages. Summer at Madeira health forms are standard across camps. If your camper will be joining us for any combination of Camp Greenway, Girls First, and/or Riding Workshop only one set of forms needs to be submitted per-camper.
