How do I obtain a CHKD medical records release form?

CHKD medical records release forms are available in English and Spanish below. You may also request a blank form by calling our health information management team at (757) 668-7764 or by emailing HIMRecordRelease@CHKD.org.

Masking Still Required

Even though we have made progress in the battle against COVID-19, the CDC and VDH still require that everyone wear a mask inside healthcare facilities. That includes all CHKD Health System locations, including our health centers and physician practices. As always, children under the age of 2 and those who cannot wear a mask safely are exempt.

Urgent Care Hours

Effective until further notice - Due to increased demand and wait times, patients must arrive at a CHKD Urgent Care center prior to 8 p.m. to be seen. We apologize for any inconvenience. Please note: CHKD Urgent Care centers do not offer rapid COVID-19 testing.
